Short-term outcomes of Oxford phase 3 unicompartmental knee arthroplasty for medial osteoarthritis of the knee

［目的］ 探讨 Oxford 第三代单髁置换 （unicompartmental knee arthroplasty, UKA） 治疗膝内侧室骨关节炎的近期疗效。［方法］ 回顾性分析 2017 年 5 月—2020 年 5 月行单侧 Oxford 第三代 UKA 158 例患者的临床资料，评价其临床及影像结果。［结果］ 所有患者均顺利完成手术，术中无神经、血管损伤等严重并发症。手术时间 （42.42±6.26） min，术中出血量 （33.40±9.67） ml。所有患者均获随访，随访时间 12～48 月，末次随访 AKSS 临床评分、功能评分、OKS 评分均较术前显著增加 （P<0.05），影像方面，末次随访 HKA 较术前显著增加 （P<0.05）。所有患者均未出现衬垫脱位、假体松动和下沉，以及膝关节外侧室 Kellgren-Lawrence 分级进展。［结论］Oxford 第三代 UKA 治疗膝内侧室骨关节炎，具有手术时间短、失血量少、住院时间短、术后快速康复等优点，可获得满意的近期疗效。

Abstract:

[Objective] To evaluate the short-term clinical outcomes of Oxford phase 3 unicompartmental knee arthroplasty (UKA) for medial osteoarthritis of the knee. [Methods] A total of 158 patients underwent unilateral Oxford phase 3 UKA for medial osteoarthritis of the knee from May 2017 to May 2020 in our hospitals. The clinical and imaging results in short term were evaluated. [Results] All patients were successfully operated without serious complications such as nerve and vascular injury, whereas with operative time of (42.42±6.26) min, and intraoperative blood loss of (33.40±9.67) ml. As time went during the follow-up lasted for 12 to 48 months, the AKSS clinical and functional scores as well as OKS score significantly increased (P＜0.05) . Radiographically, hip-knee-ankle angle (HKA) was significantly higher at the latest follow-up than that before surgery (P＜0.05) . None of the patients showed prosthetic dislocation, loosening or subsid- ence, as well as osteoarthritic progression of the lateral compartment of the knee in term of Kellgren-Lawrence grading at the latest followup. [Conclusion] The Oxford phase 3 UKA with advantages of short operation time, less blood loss, short hospital stay and rapid postopera- tive recovery does achieve satisfactory clinical outcomes in short term for medial compartment osteoarthritis of the knee.
