Combined conservative treatment for old pubic symphysis separation

摘要:

［目的］探讨陈旧性耻骨联合分离保守治疗方法的可行性。［方法］回顾性分析 2018 年 2 月—2020 年 8 月采用持续牵引下三维整复，骨盆带固定和运动疗法的综合保守方法治疗的 30 例陈旧性产后耻骨联合分离患者的临床资料。评价临床及影像结果。［结果］治疗过程中无明显不良反应，与治疗前相比，治疗后 4 周及 6 个月的 VAS 评分和 SF-36 评分各项均显著改善 （P<0.05）。治疗 6 个月时，23 例显效，6 例有效，1 例无效，总有效率 96.67%。影像方面，与治疗前相比，治疗后 4 周和 6 个月，耻骨联合距离显著减小（P<0.05）。［结论］持续牵引下三维整复，骨盆带固定和运动疗法治疗陈旧性产后耻骨联合分离临床疗效满意，具有安全性高的优点。

Abstract:

[Objective] To investigate the feasibility of combined conservative treatments for old pubic symphysis separation. [Methods] A retrospective study was conducted on 30 patients who received combined conservative therapies, including three–dimensional con- tinuous traction, pelvic band fixation and exercise therapy, for old postpartum pubic symphysis separation in our department from February 2018 to August 2020. The clinical and imaging consequences were evaluated. [Results] There were no obvious adverse reactions during the combined conservative treatments. Compared with those before treatment, the VAS score and SF-36 score were significantly improved at 4 weeks and 6 months after treatment (P＜0.05) . At 6 months after treatment, 23 patients were marked obviously effectively, 6 cases effectively and 1 case ineffective, with total effective rate of 96.67%. Radiographically, compared with that before treatment the pubic symphysis separa- tion distance was significantly reduced at 4 weeks and 6 months after treatment (P＜0.05) . [Conclusion] This combined conservative therapy, involving continuous traction for three- dimensional reduction, pelvic band fixation and exercise therapy, has a satisfactory clinical out- comes for old postpartum pubic symphysis separation with an advantage of high safety.
