    目的]探讨膝关节磁共振(magnetic resonance imaging, MRI)图像上重建断层以测量近端胫腓关节(proximal tibiofibular joint, PTFJ)倾斜角的可行性与可靠性。[方法]回顾性分析在拍摄PTFJ片的1年内进行过同侧膝关节MRI检查的患者。在PTFJ片上,测量PTFJ腓骨侧关节面相对于胫腓骨共同纵轴线的倾斜角(fibular inclination angle of PTFJ surface relative to the common longitudinal axis of tibiofibula on the radiograph, X-FIA)。软件重建MRI图像并测量PTFJ腓骨侧关节面相对于胫腓骨共同纵轴线的倾斜角(fibular inclination angle of the PTFJ surface to the longitudinal axis of both fibula and tibia on MRI, TF-FIA)、与PTFJ腓骨侧关节面相对于胫骨纵轴线的倾斜角(fibular inclination angle of the PTFJ surface to the longitudinal axis of tibia on MRI, T-FIA)。评估TF-FIA与T-FIA测量的信度。比较X-FIA与TF-FIA之间、以及X-FIA与T-FIA之间的差异。采用了Bland-Altmann分析法分别对TF-FIA与X-FIA、T-FIA与X-FIA的一致性进行评估。[结果]35例患者(35膝)纳入本研究。TF-FIA与T-FIA测量的信度为优异。X-FIA与TF-FIA之间无统计学差异(P=0.244)。X-FIA与T-FIA之间有统计学差异(P=0.000)。TF-FIA与X-FIA的95%一致性界限为-0.29±2.84°;T-FIA与X-FIA的95%一致性界限为4.61±6.69°。[结论]在膝关节MRI图像上通过校准和重建断层来测量PTFJ倾斜角是可重复的技术。TF-FIA与X-FIA可以相互替代。

    Abstract:

    [Objective] To study the feasibility and reliability of reconstructing tomograms on the knee magnetic resonance imaging (MRI) images to measure the inclination angle of proximal tibiofibular joint (PTFJ) surface. [Methods] The included population was patients who had undergone ipsilateral knee MRI within 1 year after taking PTFJ radiography. On PTFJ radiography, the inclination angle of PTFJ surface relative to the common longitudinal axis of tibiofibula (X-FIA) was measured. MRI images were reconstructed by software, and the fibular inclination angle of the PTFJ surface to the longitudinal axis of both fibula and tibia (TF-FIA) and fibular inclination angle of the PTFJ surface to the longitudinal axis of tibia (T-FIA) were measured. The reliabilities of TF-FIA and T-FIA measurements were evaluated. Compare the differences between X-FIA and TF-FIA, and between X-FIA and T-FIA. Bland Altmann analysis was used to evaluate the consistency between TF-FIA and X-FIA, T-FIA and X-FIA. [Results] 35 patients (35 knees) were included in this study. The reliabilities of TF-FIA and T-FIA measurements were excellent. There was no significant difference between X-FIA and TF-FIA (P=0.244). There was significant difference between X-FIA and T-FIA (P=0.000). The 95% consistency limit of TF-FIA and X-FIA was -0.29±2.84°; The 95% consistency limit of T-FIA and X-FIA was 4.61±6.69°. [Conclusions] It is a repeatable technique to measure the PTFJ inclination angle by calibration and reconstruction on knee MRI images. TF-FIA and X-FIA can replace each other.
