自体富血小板血浆治疗脂肪肉瘤切除后顽固性创面

Platelet-rich plasma for treatment of intractable wounds secondary to resection of liposarcoma

摘要:

[目的]介绍自体富血小板血浆（platelet-rich plasma, PRP）治疗脂肪肉瘤切除后顽固性创面的手术技术和初步临床效果。[方法] 对 12 例脂肪肉瘤切除后顽固性创面患者行 PRP 注射后缝合创面。采集患者外周血制作 PRP 10~40 ml。注射 PRP 1~2 次完全填充缺损，Ⅰ期缝合创面。术后行 B 超检查，明确深部间隙愈合情况。术后及随访观察患者伤口肉芽组织的生长情况、伤口愈合情况。[结果]10 例患者创面Ⅰ期愈合，2 例患者经二次清创，二次自体 PRP 填充后，联合 VSD 持续负压引流，均于 2 周后创面愈合。所有患者伤口均愈合较好，外观良好，无窦道形成。[结论]PRP 治疗脂肪肉瘤切除后顽固性创面可以充分填充深部间隙，促进创面愈合，能够取得满意的临床疗效。

Abstract:

[Objective] To introduce the surgical techniques and preliminary clinical outcomes of autologous platelet- rich plasma (PRP) used for intractable wounds after liposarcoma resection. [Methods] A total of 12 patients with intractable wounds secondary to resec- tion of liposarcoma underwent direct closure of the wound by suture after injection of PRP. The peripheral blood was collected, and 10 ~ 40 ml PRP was prepared. After debridement, the wound was completely filled by PRP once or twice, and closed by suture at the same stage. Ul- trasonography was performed after surgery to confirm the deep space healing, while the growth of wound granulation tissue and wound heal- ing were observed after operation and follow-up. [Results] All the 10 patients had the wound healed at stage Ⅰ, while the other 2 patients had wound healing after secondary debridement, and the second PRP filling and continuous negative pressure drainage with VSD. At the latest follow- up, all patients got good wound healing with good appearance and no sinus formation. [Conclusion] The PRP can fully fill deep space and promote wound healing in the treatment of refractory wound secondary to liposarcoma resection, and does achieve satisfacto- ry clinical efficacy.
