膝骨关节炎胫骨高位截骨联合富血小板血浆的荟萃分析△（开放获取）

A meta-analysis on high tibial osteotomy combined with platelet-rich plasma for knee osteoarthritis

摘要:

[目的]系统评价胫骨高位截骨术（high tibial osteotomy, HTO）联合富血小板血浆（platelet rich plasma, PRP）治疗膝骨关节炎的临床疗效及安全性。[方法]检索 PubMed、Embase、Web of science、CNKI、万方、维普数据库有关 HTO 联合 PRP 治疗膝骨关节炎的对照研究，采用 RevMan 5.4 进行分析。[结果]共纳入 11 项研究，共 617 例患者。荟萃分析结果显示：整体上看，HTO 联合 PRP 治疗后膝关节的 VAS 评分 （MD=-0.58, 95%CI: -0.78~-0.38, P＜0.001）、关节活动度 （range of motion, ROM）（MD=7.24, 95%CI: 2.79~11.69, P＜0.001）、WOMAC 评分 （MD=-5.78, 95%CI: -7.71~-3.86, P＜0.001）、HSS 评分 （MD= 3.15, 95%CI: 0.64~5.66, P=0.01）、Lysholm 评分 （MD=6.67, 95%CI: 5.49~7.84, P＜0.001） 均显著优于对照组。[结论] 在 HTO 对 KOA 的治疗中，联合 PRP 的疗效优于其他对照，且具有一定的安全性。

Abstract:

[Objective] To systematically evaluate the clinical efficacy and safety of high tibial osteotomy (HTO) combined with plateletrich plasma (PRP) in the treatment of knee osteoarthritis. [Methods] PubMed, Embase, Web of science, CNKI, Wanfang and VIP databases were searched for the control studies of HTO combined with PRP in the treatment of knee osteoarthritis, and RevMan 5.4 was used for analysis. [Results] A total of 11 studies involving 617 patients were included. As results of the meta-analysis on the whole, the HTO+PRP was significantly better than the control group in terms of VAS score (MD=-0.58, 95%CI: -0.78~-0.38, P＜0.001), range of motion (ROM) (MD= 7.24, 95%CI: 2.79~11.69, P＜0.001), WOMAC score (MD=-5.78, 95%CI: -7.71~-3.86, P＜0.001), HSS score (MD=3.15, 95%CI: 0.64~ 5.66, P=0.01), Lysholm score (MD=6.67, 95%CI: 5.49~7.84, P＜0.001). [Conclusion] The HTO combined with PRP does achieve considerably better consequence for the KOA over other controls, with certain safety.
