先天性髋脱位全髋置换术翻修:6例报告和综述

Revision total hip arthroplasty for congenital hip dislocation: A six-case report and literature review

本文报道了 6 例先天性髋关节脱位 （congenital dislocation of hip joint, CDH） 行全髋关节置换术 （total hip arthroplas- ty, THA）失败的患者，失败原因包括假体松动、假体周围骨折、髋臼或股骨侧骨缺损等。由于 CDH 患者本身生理结构的异常以及 THA 后的并发症，其翻修手术难度相对较大。本文中 6 例患者均通过人工智能术前规划系统辅助（AI-HIP 系统）术前设计翻修手术方案，术中均根据其具体的髋关节结构选择合适的翻修策略，术后随访均取得了满意的疗效。此外，本文通过文献回顾，着重介绍了 CDH 的结构特点、翻修原因、手术难点以及 AI-HIP 翻修规划的应用，旨在为临床提供参考。

Abstract:

This article reports six-case revision total hip arthroplasty (THA) for failed THA in patients with congenital dislocation of the hip (CDH), with the failure causes of prosthetic loosening, periprosthetic fractures, and acetabular or femoral bone defects. Due to the inherent anatomical abnormalities in CDH, the complications following THA and revision surgeries are particularly challenging. In this study, all six patients underwent preoperative planning assisted by the Artificial Intelligence Preoperative Planning System for Total Hip Arthroplasty (AI-HIP), which helped design the revision strategies tailored to their specific hip structures, and achieved satisfactory outcomes. Additionally, this article reviews the literature to highlight the structural characteristics of CDH, reasons for revision, surgical challenges, and the application of AI-HIP in revision planning, aiming to provide a reference for clinical practice.
