骨免疫对激素性股骨头坏死作用的研究进展△

Research progress of the role of bone immunity in steroid-induced osteonecrosis of femoral head

摘要:

随着糖皮质激素的广泛使用，激素性股骨头坏死（steroid-induced osteonecrosis of femoral head, SONFH）的发病率逐年升高，患者群体也更趋向于年轻化。目前，SONFH 的发病机制仍未明确，以往的研究认为 SONFH 与股骨头凝血功能障碍、 脂肪代谢紊乱、骨内压增高、氧化应激、细胞凋亡、细胞自噬、遗传因素及非编码 RNA 异常等机制有关。最新的研究发现， 骨免疫因子介导的慢性炎症可能在 SONFH 的发病机制中扮演重要的角色，慢性炎症刺激会促进 SONFH 的发生与发展。本文主要围绕骨免疫相关的免疫细胞、炎症因子及信号通路在 SONFH 发病中的相关机制进行综述，探究骨免疫因子与 SONFH 发生发展的内在联系，为防治 SONFH 提供新的思路。

Abstract:

With the widespread use of glucocorticoids, the incidence of steroid-induced osteonecrosis of the femoral head (SONFH) is increasing year by year, and the patient population tends to be younger. Currently, the pathogenesis of SONFH remains unclear. Previous studies have suggested that SONFH is associated with femoral head coagulation dysfunction, lipid metabolism disorders, increased intraos-seous pressure, oxidative stress, apoptosis, autophagy, genetic factors and non-coding RNA abnormalities. Recent studies have found that bone immunity may play an important role in the pathogenesis of SONFH, and chronic inflammatory stimulation caused by immune dysregulation promotes the occurrence and development of SONFH. In this paper, we review the mechanisms of immune cells, inflammatory factors and signaling pathways related to bone immunity in the pathogenesis of SONFH, and explore the intrinsic links between bone immunity and the development of SONFH, so as to provide new ideas for the prevention and treatment of SONFH.
